MEDICAL RECORDS INFORMATION

At our office we keep three types of medical records:

Adult Medical Record: The medical record is kept for the individual seeking treatment. At any time you may invite guests (family and friends) to join you in counseling for support or to work on relationship issues. Your guests do not have any rights to the medical record even if they attend all sessions with you.

Joint (Comingled) Medical Record: For Couples Therapy or Family Therapy – the medical record is a single comingled record of all members participating in therapy. All information and treatment notes will be placed in a joint medical record and recorded in one file. The rights to the medical record belong to the adult(s) that sign the treatment consent and client registration forms. Signed released forms from all adult(s) who signed the initial consent for treatment are required to release a couples or family therapy medical record.

Child/Adolescent Medical Record: The medical record for minors includes sessions with the minor and their parents or guardians. Both parents and/or guardians have legal rights to the medical record until the minor is age 18 regardless of who pays or attends sessions with the minor child/adolescent.
